1) Photo ID for all adults at home.
2) Birth certificate for all children at home.
3) Social security card for all at home.
4) Rental agreement with the name and address of the applicant.
5) Proof of all income for the house (including cash assistance, food stamps and maintenance).
6) Proof of all expenses - any bills that the applicant pays , eg.
