There is no need to make an appointment for the first visit, and each subsequent visit will only take place once a month.
They require a Will county photo ID, a piece of mail that verifies that address for every member of the family other then children under the age of 18.
They require a birth certificate for children under the age of 18 or a medical cards that shows the children's date of birth for the first visit only.
