The customer must have a phone available that will not block an incoming confirmation call.

Documentation Required: Proof of Address/Residency utility bill), Photo I.D., Proof of children birth certificate, social security card, health insurance card, or shot records).

Hours of operation:

3rd or 4th Saturday of the month by appointment usually from 1:00 pm to 3:00 pm.

Must make appointment by the Wednesday before the distribution date.
